Beach Wheelchair Reservation Request Form

Mobi-Chair Beach Wheelchair

  • To request the Beach Wheelchair, simply complete the registration form below.
  • Please upload your driver's license photo side only.  
  • The beach wheelchair reservation is available on a first-come, first-served basis.
  • Please submit your reservation request with a minimum notice of 72 hours. 
  • To request a reservation with less than 72 hours notice, please contact City Hall at (409) 737-1142.
  • The beach wheelchair will be reserved for the entire day and must be picked up after 8:00am and returned by 9:00pm the same day.
  • You will be contacted to confirm your reservation.
  • If the Beach Wheelchair is already reserved, you will be contacted to select a new reservation date.
  • Once confirmed, the Beach Wheelchair must be picked up and returned to:  Jamaica Beach City Hall, 16628 Termini-San Luis Pass Road, Jamaica Beach, TX 77554
